In applying for a designation the candidate certifies that to the best of their knowledge all information provided in the application herein is true and correct.  The Candidate authorizes the MIFSP to investigate the credentials and endorsements pertinent to my application and authorize the release of the same. Evidence of any license, diploma, or similar documents may be required if MIFSP has trouble verifying any portion of a Candidate's application and MIFSP reserves the right to request proof of the same be submitted following the initial application if necessary via facsimile,  mail, or e-mail.
